Affiliation with Henry Ford Health System has created opportunities for Henry Ford Allegiance Health and Jackson Health Network to collaborate with the broader system around shared opportunities in the common pursuit of the Quadruple Aim. These shared opportunities included infrastructure and technology development, learning and continuous improvement, scaling of best practices, and advancement of shared strategies for provider engagement. Collaboration included care redesign, population health management and transformation of health care financing.
Henry Ford Physician Network (HFPN) is the clinically integrated network addressing improvement of health outcomes and cost of care of in HFHS’s North and South Markets. The HFPN is comprised of physician organizations, including the Henry Ford Medical Group, along with hundreds of independent specialists and primary care physicians. Like the benefits of participation in Affirmant Health Partners, JHN’s collaboration with HFPN has provided opportunities to share learnings, cooperate on improvement initiatives, and develop tools and infrastructure that advance our mutual goals of supporting physician performance. While each network maintains separate governance and operational leadership structure, there is significant strategic alignment across the markets around the transition to value-based care.
Access to advanced information technology and analytic capabilities within HFHS has been a significant advantage to JHN. The HFHS Enterprise Data Warehouse (EDW) combines data from a variety of internal and external sources including clinical data from the Epic electronic health record and health plan eligibility, claims and other financial data. Data analytic and presentation tools interfacing with the EDW allow customized creation of dashboards and reports to provide data to clinical and operational leadership in support of the system’s strategy, performance measurement and performance improvement goals. This level of data insight, integration and analytic capability offers advanced levels of support for point of care and population-level clinical decision making. This, in turn, allows JHN to better convey real-time performance opportunities and improvements to providers, payors, employers, and other clients and stakeholders.
Affiliation with HFHS also connects HFAH and JHN with their subsidiary provider-sponsored health plan, Health Alliance Plan (HAP). HAP is a full-service health insurance company with various product portfolios for individuals, companies and organizations, serving over half a million members across 23 counties in Michigan. While JHN maintains successful contractual relationships with a variety of commercial and governmental payors, collaborating with HAP in an integrated delivery system model creates additional opportunities to care for our populations more seamlessly and comprehensively through coordination of payor and provider services, such as care management.
