Donna Eberly-Paquette, Practice Transformation/Program Specialist
For the past 10 years, Donna has worked in health care as a practice manager, a biller, a lead medical assistant and most recently a health care transformation representative. Her dedication and hard work has helped primary care practices earn Patient Centered Medical Home designation through BCBSM. She understands the importance of workflows and does whatever it takes to assist practice managers in finding the best solution for their team. Donna has a Bachelor's Degree in Health Services Administration.

Brittany Easton, Practice Transformation/Program Specialist
Brittany is a lifetime resident of Jackson. Her experience in healthcare started at a primary care office as an office support person for 3 years before continuing her education at Grand Valley State University where she received a degree in Public and Nonprofit administration and community health. Brittany also completed an internship in the Medical Practice Management Department at Allegiance Health where she gained knowledge in office performance, management, and insurance guidelines.

Ali Aldridge, Practice Transformation/Program Specialist
Ali earned her Bachelor's degree in Healthcare Administration from the University of Michigan. She comes to Jackson Health Network with over six years of healthcare experience. Her most recent experience with population health included work related to the Patient Centered Medical Home Model, Blue Cross Blue Shield Physician Group Incentive Program, and Physician Quality Reporting System.

Laurie Tarpley, Practice Transformation Specialist
Laurie comes to JHN with more than 20 years experience working in the health care field. For the past 15 years, Laurie worked for various health plans, including Physician’s Health Plan of South Michigan, Priority Health, and Meridian Health. Her office management in primary and specialty care has given her insight into the functioning of offices and impact of pay-for-performance systems on daily functioning. Laurie provides principal support to the offices of our primary care members.

Sherry Walling, Practice Transformation Specialist
Prior to Sherry joining the JHN team, she managed a local pediatric practice for 16 years and most recently spent a year working as a health care transformation representative at Jackson Physicians Alliance. Sherry's experience as a practice manager brings great skills to the JHN team. She understands the day to day problems that arise in a practice setting and can help offer solutions using her own experience. During her time spent at JPA, she focused on helping all primary care offices improve their HEDIS indicators, increase patient compliance, and earn PCMH designation.

Care Coordination Staff
Nancy Arce, Supervisor, Care Coordination
Nancy has been a Licensed Master “Clinical” Social Worker for more than 20 years and has served in a multitude of capacities throughout her career providing services to those with mental health, medical and substance abuse problems. Nancy has been with Allegiance Health since 2001. During this time she has worked as both a Quality Improvement Specialist and Network Services Supervisor for Physician’s Health Plan of South Michigan (PHPSM), but comes directly to JHN after having served for 6 years as Case Manager in the Emergency Department. Her experience with arranging/coordinating services for persons of various ages and diseases will be a significant bonus to our Care Coordination processes.

Gail Brenton, Care Coordinator
Gail is a Licensed Master’s Social Worker and comes to JHN after 24 years as a therapist in Behavioral Health. She worked for Allegiance Substance Abuse Services (ASAS, formerly Bridgeway Center) for 20 years prior to transferring to JHN. During her social work tenure, Gail assisted those adjudicated for alcohol or drug-related legal issues as well as persons struggling with Co-occuring Disorders (substance use disorders plus mental health diagnoses) through assessments, individual, group therapy, and treatment/discharge planning. She also developed and facilitated a Women’s Recovery group while at ASAS and served on the Jackson County Recovery Court multidisciplinary team for 10 years. Gail is excited to be a member of the Care Coordination team! Undoubtedly, her varied skill set will be essential to integrating mental health into the overall population health experience.

Michelle Cornstubble, Care Coordinator
Michelle is an Registered Nurse who earned her Bachelors of Applied Science degree through Siena Heights. Michelle has a wide variety of nursing experience within Allegiance Health ranging from Staff Nurse, Educator, and Employee Health Nurse. She was also the Neuromuscular Service Navigator at the Allegiance Specialty Center; assisting patients who needed access to medical care.
Her experience also includes being the Director for Ganton Home Care, responsible for opening and running the business. Michelle’s most recent experience is as a Nurse Liaison for Mary Free Bed and Sparrow coordinating admissions to Acute Inpatient Rehabilitation for trauma and stroke victims. Michelle is compassionate and dedicated to helping people, which is evident when you speak with her.

Elizabeth Mangold, Care Coordinator
Elizabeth is a registered nurse who graduated from Michigan State University with a Bachelor of Science in Nursing. She spent nearly four years as a staff nurse at Allegiance Health working primarily with stroke, cardiac and urinary patients. Elizabeth has also taught nursing clinical groups from Jackson College for three semesters on several med/surg units. Additionally, she completed a Six Sigma Green Belt project which successfully decreased the incidence of Post-Operative Respiratory Failures. Her approach as a nurse is informed by the relationship based care model; she believes that understanding the unique needs and life stories of patients is crucial to successfully partner with patients in their journey to better health. Elizabeth's experience will be an great asset to JHN's care coordination program.

Rita Potts, Care Coordinator
Rita is a Registered Nurse who earned her Bachelors of Science in Nursing through Grand Valley State University. Rita has many years of experience in the healthcare field. Prior to becoming a Nurse, she worked as a medical assistant for a primary care office for 5 years. After graduation, Rita became a staff nurse for an Allergy and Immunology office in Ann Arbor for 3 years, and was an Integrated Healthcare Nurse for Community Mental Health in Adrian for 10 years providing nursing care for mental and behavioral health, as well as substance abuse and co-occurring disorders. Rita also worked for an attorney group, completing medical case reviews, preparing them for litigation. With her extensive experience working with patients with vast medical and socioeconomic issues, Rita is an asset to JHN and the community. She is dedicated to helping people gain knowledge and independence to successfully live healthier lives.

Christine Rhoades, Care Coordinator
Chris has been a Registered Nurse for more than 25 years and brings a wealth of knowledge to the team. Chris has experience in: Home Care; Public Health Nursing (Maternal/Child Health Programs, Communicable Disease); Discharge Planning for the elderly; and has worked as a Medical Case Manager for MESSA with catastrophically ill patients. Prior to her Care Coordination role she served as a Utilization Management Coordinator for Allegiance Health's Case Management department. Her vast experience will help patients in making smooth transitions across the care continuum.

Nicole Gargano, Care Coordination Pharmacist
Nicole received her Doctorate of Pharmacy degree from Ferris State University and completed her pre-pharmacy studies at Michigan State University. She comes to JHN with extensive retail pharmacy experience as both a staff pharmacist and pharmacy manager. Most recently, Nicole was a staff pharmacist at the Allegiance Health Ambulatory Care Pharmacy. Additionally, Nicole has worked with refugee families, helping to navigate the American health care system as well as understand their medications and the importance of medication adherence and compliance. Her experience with providing comprehensive medication reviews, as well as optimizing medication regimens will be a great asset to JHN and the care coordination program.

Kathi Tennis, Supervisor, Transition Coordination
Kathi received her bachelor’s degree in nursing from Northern Michigan University in 1977. Her past experiences include CCU, Mother/Baby, ED and Med/Surg at a small hospital in Newberry, MI, then Director of Nursing at a Nursing Home (also in the Upper Peninsula). She was a school nurse for 5 years and has been the Clinical Supervisor for Allegiance Personal Care and TeleHealth/Telemonitoring for the last 15 years. In 2008, Kathi was part of the HIS team that received the Most Wired Innovator Award for the discharge phone call and rule based continuity of care initiative.

Michelle Long, Transition Coordinator
Michelle has been a Registered Nurse for the past 15 years. During her nursing career, she has worked at the Center for Family Health with their pediatric and internal medicine patients, and was a school nurse here in Jackson for 9 years. She is also an instructor at Lansing Community College and teaches both the skills lab and clinical sites for CNAs. Additionally, she was the Personal Care Clinical Nurse Coordinator for 2 years prior to being a TeleHealth Nurse.

Jacquie Melling, Transition Coordinator
Jacquie received her ADN from Jackson Community College in 1994 and then her BSN from Ferris State University in 2007. She began her career as a floor nurse at Foote Hospital for 11 years in the oncology/renal floor. She then transferred to Allegiance Hospice where she worked for 8 years as a field nurse before transitioning to the In-Home Hospice Educator for almost 3 years. Jacquie began her job as Transition Coordinator for JHN in February 2016. In addition to her part-time position within Henry Ford Allegiance, she has also been the school nurse at Hanover-Horton for the past 17 years. Jacquie and her husband Chris are raising their 7 children in the Hanover-Horton community where they both grew up.

Regina Surbrook, Transition Coordinator
Regina received her ADN from Jackson Community College in 1996 and her BSN from Eastern Michigan University in 2009. She began her career as a CENA and medical secretary while attending nursing school and transitioned to a floor nurse and eventually a Supervisor at Doctors Hospital in Jackson on the Medical/Surgical Unit, Step-down unit and ICU, working for a total of 9 years at that hospital until 2003 when the hospital closed. Regina moved to Florida the same year and worked for HCA as a Charge Nurse on PCU and Supervisor at Englewood Community Hospital, a Supervisor at Doctors Hospital in Sarasota and ADON at Edward White Hospital in St. Petersburg. In 2007, Regina returned to Michigan and worked for Chelsea Hospital on the Head Pain Unit. In 2008, Regina started with Hospice of Jackson (Allegiance Hospice) and worked in several roles providing care to patients and their families, working in the role of supervisor and in 2011, accepting the role of manager for the In Home Hospice program until 2015. Regina was working as a RN medical coder in the Intake department for Home Care, Hospice and Personal Care just prior to starting the role of Transition Coordinator with JHN.

Kristin Jervis, Program Performance Coordinator
Kristin brings five years of experience in working with medical offices to review, analyze, and identify opportunites in performance. Kristin has a great eye for detail and is able to work with offices to ensure success in the Jackson Health Network quality programs. Kristin has earned a Master's degree and Bachelor's degree in Business Administration, both of which are a great asset to the role she plays at JHN.

Jess Cox, Data and Program Technician
Jess brings over 16 years of experience in both medical billing and administrative assistance in the healthcare industry to Jackson Health Network (JHN). Jess’s knowledge of healthcare and assisting practices with collecting and entering patient data is a great asset to JHN and contributes to creating a healthier community.

Jia Li, MS, ME, Performance Measurement Analyst
Jia Li has joined the CI team as our new JHN Performance Measurement Analyst. Jia received her undergraduate degree in InformationEngineering from Beijing University. She also has 2 Master’s degrees in Electrical Engineering and Engineering Management from Syracuse University. Before coming to JHN, Jia spent a year as a developer with Jackson County Medical Records (JCMR) and brings significant expertise in data analysis, database management and knowledge of Electronic Health Records to JHN.
